Requirements:

• Active Washington D.C. License & active Compact/Multistate License (LPN).
• Technical aptitude in Microsoft Office Suite.
• Excellent written and verbal communication skills.
• Epic Experience and bilingual skills are a plus.
• Additional state licensures (LPN) are advantageous.

Responsibilities:

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care.
• Identify and coordinate community resources with patients that would benefit their care.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management, including identifying concerns, reconciliation, adherence, and coordinating refills.
• Assist in ensuring timely delivery of services like Home Health, DME, and Home Infusion.
• Resolve patient inquiries and manage referrals and appointment scheduling.
