Requirements:

• A clinical licensure is required (e.g., LCSW, LPC, LMFT, Licensed Psychologist).
• Experience managing a team and implementing programs (3-5 years).
• Master's degree in behavioral health, social work or health care management.
• Knowledge of laws, regulations, and ethical standards in care delivery.
• Strong relationship management and problem-solving skills.
• Excellent communication and collaboration capabilities.

Responsibilities:

• Act as a player/coach, supporting Care Extenders and members.
• Assess member risk and implement crisis interventions.
• Create personalized service plans and support members' skill development.
• Monitor members' progress and coordinate communications.
• Ensure compliance with performance indicators and documentation.
• Conduct interviews, training, and performance reviews for team members.
