Requirements:
  • Must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree.
  • Licensed Pharmacist in the state of Oregon required.
  • 1+ years' experience strongly preferred but not required.
  • Ability to identify barriers to medication adherence and implement improvement plans.
  • Strong verbal and written communication skills.
  • Knowledge of specialty medication pharmacology and local/state/federal pharmacy regulations.
  • Proficient in Microsoft Word, Excel, and PowerPoint; experience with EPIC/QS1/WILLOW preferred.
  • Strong technical and clinical skills; ability to motivate and coach patients.
Responsibilities:
  • Provide integrated support to patients and physicians via personalized relationships.
  • Review clinician's notes for supportive clinical data for prior authorization approvals.
  • Develop disease and drug specific clinical protocols for patient adherence.
  • Monitor patient adherence using various data sources, focusing on oncology and specialty diseases.
  • Evaluate EMR for medication selection, interactions, and optimize patient outcomes.
  • Recommend alternative drug regimens per guidelines.
  • Monitor therapies for side effects and propose therapy changes.
  • Conduct medication therapy management and document interventions.
  • Perform clinical interventions including pharmacist consultations and medication synchronization.
  • Oversee tech's daily activities for accuracy in delivery and prior authorization.
  • Identify and resolve insurance issues and ensure HIPAA compliance.
  • Engage in continuous education and share knowledge with pharmacy staff.
  • Assist with research projects and peer review as necessary.
