🔍 Mental Health Services
- Master’s degree in Social Work or a related field.
- Valid and current LCSW or LMHC license.
- Minimum of 2 years of clinical social work or mental health counseling experience.
- Strong communication and listening skills.
- Ability to conduct psychosocial assessments and develop comprehensive care plans.
- Digitally proficient with tools such as Google Workspace, Excel, Zoom, and EMR systems.
- Bilingual (English/Spanish) preferred.
- Conduct remote assessments to evaluate patients' mental health needs and develop personalized treatment plans.
- Provide individual counseling sessions to address emotional, behavioral, and social challenges.
- Collaborate with medical and internal teams to coordinate patient care and ensure comprehensive support.
- Offer crisis intervention and support during acute distress or emergency situations.
- Maintain accurate, confidential patient records in compliance with HIPAA and company regulations.
- Continuously monitor and evaluate patients' progress, adjusting treatment plans as necessary.
- Educate and empower patients and their families to manage mental health concerns effectively.
- Participate in regular professional development activities and internal meetings with cross-departmental teams.
