Requirements:
- Bachelor's degree, preference for global health, health management, or business.
- Minimum of five (5) years related clinical experience, preferably in value-based kidney care or population health.
- Preferred: Master's degree in nursing, business, health services research, or relevant field.
- Knowledge of strong interpersonal/customer service skills.
- Problem-solving and analytical abilities.
- Ability to use electronic population management systems after training.
- Good organizational skills, multitasking, and prioritization capabilities.
- Intermediate skills with Microsoft applications including Word, Excel, and PowerPoint.
Responsibilities:
- Manage the total cost of care and clinical quality for your market under guidance of an SVP of Operations.
- Assess opportunities within markets and target specific cost savings via evidence-based protocols.
- Drive development and implementation of population health initiatives.
- Identify gaps in CKD and ESKD management and establish relationships with local providers.
- Direct initiatives focusing on health equity across communities and utilize technology for scalability of care.
- Monitor KPIs for initiatives to evaluate success and support care management plans.
- Conduct research for best practices to enhance CKD/ESKD patient care.
