Requirements:
  • Minimum of 6 months previous experience as a Renal Care Coordinator with Interwell Health.
  • 1-2 years of prior RCC experience is highly preferred.
  • A combination of renal transplant, dialysis, or CKD patient care is required.
  • Strong organizational and communication skills are necessary.
  • Must be available to work during the hours of the practices supported.
Responsibilities:
  • Assess patients' understanding of late-stage CKD and treatments to facilitate informed decision-making.
  • Provide support and guidance for patients exploring conservative or palliative care options.
  • Act as a liaison to ensure patients receive comprehensive information on treatment modalities and education.
  • Organize patient education regarding CKD and coordinate a stable transition to renal replacement therapy.
  • Utilize motivational interviewing techniques to encourage patient engagement in their care.
  • Participate in interpreting clinical data to improve CKD care processes.
