Care Coordinator - East Coast

Requirements:

• 3 years of experience in healthcare coordination, community health work, or similar healthcare-related field.
• Experience with a team-based approach to care.
• Excellent knowledge of community resources.
• Exceptional ability to prioritize and task-switch.
• Self-directed and motivated to resolve issues.
• Strong communication and teaching skills.
• Intermediate skills with MS Office Suite including Outlook and Teams.
• Ability to work in a primarily remote environment with necessary internet speed requirements.

Responsibilities:

• Perform telephonic outreach to enroll and engage patients for clinical programs.
• Develop and implement care plans under the direction of the Nurse Care Manager.
• Facilitate transitions of care and update patient care plans as necessary.
• Conduct assessments to identify Social Determinants of Health and recommend appropriate resources.
• Guide members in understanding health services and advocate for member needs.
• Maintain open communication with patients and healthcare teams.
• Ensure accurate documentation of interactions and assessments.
• Provide educational resources to empower patients.
• Other duties as assigned.
