Requirements:

• MD or DO degree and Board Certified or Board eligible in Palliative Care, Hospice & Palliative Medicine, or a related field.
• Active, unrestricted medical license to practice in New Jersey.
• Active DEA and CDS licenses.
• Minimum of 3-5 years of clinical experience in palliative care, hospice, or a related field.
• Preferred experience in home-based care and value-based care settings.
• Desirable previous experience in starting, managing, or growing a healthcare practice.

Responsibilities:

• Provide comprehensive palliative care to patients in their homes, focusing on pain and symptom management and improving their quality of life.
• Conduct in-depth assessments and develop personalized care plans, managing the medical needs of patients with serious conditions.
• Establish goals of care with patients and families, emphasizing patient autonomy.
• Collaborate with a multidisciplinary team to ensure holistic patient care.
• Develop partnerships with healthcare providers to expand service reach.
• Train and manage additional healthcare staff as the practice grows.
• Implement quality assurance processes and collaborate with leadership on operational improvements.
