Palliative Care Nurse Practitioner (NP) - North Jersey

Requirements:

• You are based in Northern New Jersey (required).
• You have a BSN from an accredited program AND a master's degree in advanced clinical practice nursing or a post Master’s certificate (required).
• You have worked as part of an in-patient palliative care team and/or as a hospice RN (required).
• You have an active Nurse Practitioner license to practice in the state of New Jersey (required), board certified in Palliative Care or Hospice Nursing (preferred).
• You have an active DEA and CDS registration (required).
• You have worked in a housecalls setting previously (preferred).
• You enjoy working with patients of diverse social, ethnic, and cultural backgrounds.

Responsibilities:

• Be accountable for the care and outcomes of a cohort of supportive care patients.
• Collaborate closely with the Palliative Care Physician.
• Visit patients at home and conduct telehealth visits.
• Participate in Interdisciplinary Team Meetings.
• Contribute medical expertise to develop care plans.
• Achieve high performance on quality metrics.
• Engage with other providers in the patient care team.
• Facilitate consultations and care arrangements based on patient goals.
• Complete timely documentation of clinical encounters.
• Support continuous quality improvement efforts.
• Be part of the on-call rotation.
