Requirements:

• 3+ years' experience with population health, care coordination, or community-based programs.
• Ability to travel up to 50% within the assigned region with a personal vehicle.
• Prior Managed Care Experience is preferred.
• Excellent interpersonal and strong communication skills.
• Bachelor's Degree or higher in a related field or equivalent experience.

Responsibilities:

• Develop and implement strategies and policies for operations and projects.
• Help business units meet corporate objectives and compete in the marketplace.
• Partner with business owners to optimize strategic business objectives.
• Interact with external vendors and consultants in initiative development.
• Implement and scale SDoH strategies for members and communities.
• Utilize local knowledge to build community partnerships and pilot new initiatives.
