Requirements:

• Must have CDCES certification.
• Minimum of 3 years of experience as a Registered Nurse (RN) with active practice within the past year.
• Hold an active, unencumbered multi-state compact license to practice.
• Demonstrate cultural competence to cater to diverse backgrounds.
• Exhibit a passion for high-quality patient care aiming to improve health outcomes.
• Ability to work in a fast-paced environment with critical thinking skills.
• Work independently and make clinical decisions.
• Strong written and verbal communication skills.

Responsibilities:

• Provide telephone-based care to patients in Perry Health’s RPM program.
• Deliver swift, empathic care to members and coach them on condition management.
• Coordinate member care with internal and external providers.
• Educate members on managing their condition.
• Interpret blood glucose readings and assess member needs.
• Drive reductions in A1c through medication adjustments and education.
• Triage real-time clinical alerts from connected devices.
