Requirements:

• Must have an active and unrestricted LMSW or LCSW in state of residence.
• Minimum of 3 years experience in Home health, SNF, and/or rehab is preferred.
• Knowledge of managed care and utilization review requirements.
• Strong skills in time management, organization, negotiation, and communication.
• Ability to work in changing business environments and manage local community based resources.
• Strong personal computer skills and EMR experience.

Responsibilities:

• Collaborate with interdisciplinary teams including physicians, nurses, and rehabilitation specialists to ensure cohesive patient care.
• Develop and implement discharge plans in coordination with the treatment team.
• Monitor patient progress and adjust care plans to ensure continuity.
• Facilitate education for patients and families regarding resources and care plans.
• Participate in case conferences for patient status discussions.
• Initiate contact with patients, families, and providers to discuss treatment alternatives.
• Coordinate with external agencies for necessary services.
• Balance business needs with patient advocacy.
