Requirements:

• Active and unrestricted RN Licensure in Ohio.
• Minimum of 2 years of clinical experience.
• Discharge planning experience preferred.
• Proficient in Microsoft Office (Word, Excel, Outlook).
• Associate's Degree or Nursing Diploma required; Bachelor's Degree preferred.

Responsibilities:

• Conduct telephonic assessments and evaluations of members' medical needs.
• Coordinate service strategies and policies to enhance member wellness.
• Develop proactive courses of action to improve short and long-term outcomes.
• Utilize clinical tools to evaluate member's needs and benefit eligibility.
• Consult with supervisors and present cases at multidisciplinary conferences.
