Requirements:
  • Must have an active, unencumbered California state license.
  • RN Compact (multi-state) license in addition to a CA preferred.
  • Three (3) years RN experience.
  • Med/Surg or nursing experience in the inpatient hospitalized setting required.
  • Clinical knowledge and ability to promote patient safety and a positive patient experience.
  • Flexible and comfortable with a changing work environment.
  • Ability to work independently and as a team player.
  • Participates in ongoing learning and mentoring others.
Responsibilities:
  • Utilize monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home.
  • Navigate patients and families across the continuum of the program’s phases of care.
  • Monitor and respond to biometric data per program protocols.
  • Conduct home and audio/visual patient care visits as directed by the care team.
  • Ensure all interventions in the patient’s care plan are implemented efficiently with a patient-centered approach.
  • Leverage team resources and expertise to support safe, reliable, high-quality care.
  • Adhere to clinical protocols and participate in necessary training.
  • Promote professional practice and a culture of safety.
  • Obtain required certifications.
