Requirements:
  • A baccalaureate degree from an accredited college or university, including or supplemented by twelve semester credits in health education or in health, social or biological sciences.
  • A baccalaureate degree and six months of full-time satisfactory experience in health promotion or disease intervention/prevention performing specified tasks.
  • A four-year high school diploma or equivalent and four years of full-time satisfactory experience in a related field.
  • Education and/or experience equivalent to the above requirements.
  • All candidates must have a four-year high school diploma or its equivalent.
Responsibilities:
  • Identify appropriate methods for reaching parents and encouraging them to follow-up on child's failed screening.
  • Obtain updated contact information from schools for students who fail vision screenings.
  • Conduct outreach to parents for students referred to ensure connection to community care.
  • Complete referral forms legibly and ensure letters to parents are attached.
  • Enter priority E12S forms and return incomplete forms to supervisor, sending copies to school nurses.
  • Enter non-priority forms from assigned borough after priority forms are completed.
  • Accurately and timely enter data into Automated Student Health Record (ASHR).
  • Prepare paperwork for filing.
  • Verify prescriptions for accuracy and ensure eyeglasses match records.
  • Coordinate and support eyeglass delivery to schools.
