Requirements:

• Holds an active, unencumbered Advanced Practice license.
• Fluent in Spanish.
• Credentialed in adult, family, or geriatric care.
• Has an active DEA license.
• Preference for Developmental Disabilities Nurse certification.
• Experience in the care of adult, chronically ill patients.
• Two or more years of practice experience with adult and chronic conditions.
• Working knowledge of computers and ability to document effectively in an electronic system.
• Preference given to NPs with active DEA, NPI, and Medicaid number.

Responsibilities:

• Provide compassionate care to diverse patients and their families.
• Manage the care of individuals with various health issues ranging from primary to urgent care.
• Collaborate with the interdisciplinary team to ensure a continuum of care.
• Deliver cost-effective, high-quality care, and perform follow-ups to monitor chronic conditions.
• Handle unplanned, urgent patient contacts and adjust visit schedules as needed.
• Address Advance Care Planning and manage medication therapy effectively.
• Communicate with the care team, provide patient education, document encounters, and participate in case conferences.
