Requirements:

• Must have an active, unencumbered Oregon state license.
• RN Compact (multi-state) license in addition to OR preferred.
• Three (3) years RN experience, specifically in Med/Surge or inpatient hospital settings.
• Flexible and comfortable with a changing work environment and schedule.
• Participates in ongoing learning and continuing education for various patient populations.
• Able to promote patient safety and positive patient experience.
• Ability to collaborate, advocate, and mentor with a professional demeanor.

Responsibilities:

• Utilize monitoring, assessment, planning, intervention, and critical thinking skills to care for acute care patient populations in the home.
• Navigate patients and families across the continuum of the program’s phases of care.
• Monitor and respond to biometric data per program protocols.
• Conduct home and audio/visual patient care visits as directed by the care team and/or protocol.
• Ensure all interventions in the patient’s care plan are implemented efficiently with a patient-centered approach.
• Leverage team and supplier resources to implement collaborative workflows, standards, and documentation systems.
• Adhere to clinical protocols and acquire training for evidence-based care.
• Promote a culture of safety and engage in process improvement efforts.
• Obtain necessary certifications as required by Hicuity Health.
