Requirements:
  • Master’s Degree in Social Work (MSW) required; LCSW preferred.
  • 8+ years of experience in healthcare social work, with at least 3 years in a leadership role.
  • Highly preferred dementia and/or palliative care experience.
  • Experience working in and leading interdisciplinary care teams within a healthcare setting.
  • Strong understanding of care planning, social determinants of health, and patient advocacy.
  • Proven ability to develop and implement social work programs.
  • Exceptional leadership, communication, and team-building skills.
  • Knowledge of healthcare regulations and best practices related to social work and care coordination.
  • Ability to work collaboratively with medical, clinical, and operational teams.
Responsibilities:
  • Supervise a remote team of Social Workers.
  • Promote a positive work experience and professional growth for the Social Worker team.
  • Ensure effective daily operations of the team from technical, operational and quality perspectives.
  • Oversee quality assurance and documentation improvement plan.
  • Support development and implementation of innovative social work solutions for the dementia population.
  • Identify process improvement opportunities.
  • Ensure all clinical quality metrics are met or exceeded.
  • Assist in creation of partner reports and communications.
  • Effectively collaborate with all Rippl departments as the Social Work subject matter expert.
  • Establish a psychologically safe environment for the team.
  • Partner with Learning & Development for ongoing learning and development of the team.
  • Foster a culture of collaborative communication.
  • Monitor patient and caregiver satisfaction and address concerns.
