Requirements:
  • Registered Nurse with a New York State license AND a compact license (required).
  • Bilingual and fluent in both English AND Spanish (required).
  • 4+ years of nursing experience within acute care (required).
  • 2-3 years of experience leading/managing a clinical team of at least 15+ reports (required).
  • Familiarity with managed care plans/structure in New York State (required).
  • Experience managing a remote team (preferred).
  • Detail- and process-oriented with excellent communication skills.
Responsibilities:
  • Provide leadership, coaching, and development to a multidisciplinary team of individuals performing care management.
  • Assist in evaluating capacity planning, hiring, and training of new employees.
  • Manage, analyze and deliver productivity measures to the team and higher management.
  • Collaborate with other Managers of Clinical Delivery to ensure appropriate staffing.
  • Assist in implementing and educating new clinical programming across clinical pods.
  • Provide clinical oversight and support the team in addressing escalated member challenges.
  • Identify inefficiencies and opportunities for quality improvement.
  • Track, analyze and manage key performance indicators (KPIs) and metrics.
  • Serve as a subject matter expert for chronic care management, transitions of care, and remote patient monitoring.
  • Support the development of scaling strategies for the program.
  • Collaborate with cross-departmental leads to drive efficiencies and quality improvement.
