Requirements:

• Active New York Licensed Nurse Practitioner or Physician Assistant license with graduation from an accredited health professions school.
• Active DEA licensure and state-specific controlled substance registration.
• Certification in basic life support (BLS).
• Clinical experience caring for aging adults with complex care needs.
• Ability to travel as needed to locations with limited public transportation access.

Responsibilities:

• Provide compassionate, individualized, and holistic patient care including comprehensive care, urgent care, and chronic condition management.
• Collaborate with the multidisciplinary team to support transitions of care and complex medication management.
• Deliver exceptional clinical care both in-person and virtually.
• Maintain comprehensive medical records and timely documentation.
• Participate in team meetings and engage with internal and external partners.
