Requirements:

Must reside within Maricopa County, Arizona.
Unencumbered Behavioral Health clinical license in Arizona (e.g., LPC, LISAC, LCSW, LAC, LMSW).
3+ years of direct clinical practice experience in behavioral health.
2+ years of Case Management experience.
2+ years’ experience working with individuals with serious mental illness (SMI).
Willing to travel locally up to 10% of the time.

Responsibilities:

Conduct comprehensive evaluations of members' needs and issues.
Coordinate care plans and monitor progress in collaboration with clinic staff.
Utilize a holistic approach to overcome barriers and present cases for multidisciplinary review.
Secure appropriate services necessary for members' healthcare needs.
Empower members to make informed healthcare decisions through coaching and support.
