Requirements:

• Must have active and unrestricted RN licensure in the state of Mississippi OR compact RN licensure in state of residence.
• 3+ years of clinical experience required.
• 2+ years of Case Management, Discharge Planning, or Home Healthcare Coordination experience required.
• Preferred: Certified Case Manager.
• Excellent analytical and problem-solving skills.
• Bilingual desired.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently.
• Effective computer skills including navigating multiple systems.
• Proficiency with MS Word, Excel, Outlook, PowerPoint, and other proprietary applications.

Responsibilities:

• Responsible for assessing, planning, implementing, and coordinating case management activities.
• Develops proactive care plans to enhance member wellness.
• Uses clinical tools to evaluate member needs and benefits.
• Applies clinical judgment to address risk factors impacting care.
• Conducts assessments considering various information sources.
• Consults with supervisors and participates in case conferences.
• Utilizes motivational interviewing for member engagement.
