Requirements:

• Must be licensed as LCSW, LMFT, or LMHC in Washington State.
• Preferred experience working with a substance abuse population.
• Background in maternal substance abuse is preferred.
• Experience with both adults and adolescents required.
• Familiarity with Medicare and Medicaid procedures.
• Previous remote work experience is required.

Responsibilities:

• Assesses, plans, and implements individualized care strategies for members.
• Collaborates with members, families, and healthcare providers on care plans.
• Conducts individual counseling and group therapy with adolescents and adults.
• Identifies and initiates referrals for social service programs.
• Advocates for members' needs within the healthcare team.
• Documents assessments, care plans, and referrals.
• Participates in interdisciplinary team meetings and provides information for care transitions.
• Maintains punctuality and attendance.
