Requirements:
  • A current Medical Assistant (CMA) certification is required.
  • Fluency in English and Spanish, Russian, or Cantonese/Mandarin is required.
  • At least two years of experience as a medical assistant with at least one year experience in medication reconciliation is required.
  • Ability to work Monday to Friday, 9:00 am to 6:00 pm EST, and rotating holiday shifts is required.
  • Knowledge and understanding of chronic care management processes is required.
  • Comfort using technology like Google Workspace, multiple EMRs, and Slack is required.
  • Experience working with multiple platforms to provide a seamless experience for patients is required.
  • Capacity to be focused and productive while working from home with a private area and reliable internet connection is required.
  • Positive attitude and enjoyment in talking to patients.
  • Ability to work effectively as a member of an interdisciplinary team with good judgment and decision-making skills.
  • Ability to perform duties as assigned or requested.
Responsibilities:
  • Conduct patient interviews and create accurate, comprehensive medication lists.
  • Coordinate clinical service visits between pharmacists and members and/or caretakers.
  • Provide practice support including contacting members, caregivers, and care team members as directed.
  • Work closely with the clinical team to improve the health and care of our members.
  • Coordinate care for members of the program.
  • Enter data within operating dashboards, reporting and workflow platforms.
  • Ensure call resolution by discussing the purpose of call, effectively addressing concerns, and escalating calls as necessary.
  • Manage challenging member and/or caretaker situations and respond promptly to service requests.
  • Embrace a continuous quality improvement approach and communicate improvement ideas to the clinical services team.
  • Participate in other activities as assigned.
