• Active Certification as a Medical Assistant from an accredited association.
• 5+ years of experience as a Medical Assistant.
• Stable wifi with strong upload and download speeds for remote work.
• Highly skilled in behavioral-based coaching.
• Excellent clinical acumen and competency in patient assessment.
• Strong written, verbal, and interpersonal communication skills.
• Tech-savvy and capable of working with multiple systems.

Responsibilities:

• Support delivery of healthcare by performing activities under the direction of the Cadence Nurse Practitioner.
• Provide one-on-one coaching for patients managing chronic conditions.
• Conduct comprehensive assessments of patients' health status and readiness to change.
• Help patients execute personalized care plans focusing on behavior modification and self-management strategies.
• Monitor patients' progress and health outcomes through regular check-ins.
• Educate patients on disease management and prevention strategies.
