Requirements:

• Passion for Hazel Health’s mission.
• Certified Physician Associate or Nurse Practitioner with a license to practice in two of the following states: CA, FL, TX.
• Multi-language ability a huge plus - Spanish, Haitian Creole, or Vietnamese preferred.
• Excitement for building a new healthcare model.
• Deep clinical knowledge and expertise.
• Experience treating patients in various settings (office/clinic, video, phone).
• Experience providing both acute and primary care.
• Strong communication skills across diverse populations.
• Telemedicine experience strongly preferred.
• Strong work ethic and high integrity.
• Ability to problem solve through innovation.
• Understanding of startup culture.
• Technological skills for learning new software and completing digital paperwork.
• Available for part-time, 10 month employment (school-year based).

Responsibilities:

• Provide virtual health care services to students in partner school districts, including informing families and schools.
• Collaborate with care team members including Physicians, Clinical Support Specialists, and Family Resource Navigators.
• Utilize video telemedicine platform to evaluate and treat patients.
• Analyze and interpret patient histories and symptoms to develop diagnoses and care plans.
• Communicate with patients, families, and school personnel through various means.
• Build trust with school nurses and health staff.
• Provide training and continuing education to clinicians.
• Deliver individualized health education and guidance.
• Serve as the Pediatric care expert and share best practices.
• Support students with chronic conditions and ensure access to care.
• Provide referrals and follow-up care as needed.
