Requirements:
  • Certified and licensed as a Nurse Practitioner in good standing in the state of New York (required) and Massachusetts and/or Pennsylvania (preferred).
  • Master’s or doctoral degree from an accredited institution for nurse practitioners (required).
  • Medicare participation for billing (required).
  • Certification from ANCC (or equivalent) in relevant nursing specialties (required).
  • 1+ years of Nurse Practitioner experience (required), preferably with independent practice eligibility.
  • 1+ years of telephonic triage or equivalent experience (required).
  • 2+ years of clinical experience with complex adult populations (required).
  • Ability to practice independently with minimal support (required).
  • Comfort using technology including Google Suite and EMRs (required).
  • Experience in home care or geriatrics (preferred).
Responsibilities:
  • Triage inbound calls from members and assess actions to be taken.
  • Conduct care coordination and identify cost-effective treatment.
  • Utilize clinical skills in physical assessment for at-risk adults.
  • Provide education to members, caregivers, and facilities.
  • Monitor quality of care and update care plans.
