Requirements
- Bachelor’s degree or equivalent work experience.
- Experience in social work or case management in disability or social services.
- Experience managing multiple client relationships simultaneously.
- Excellent written and verbal communication skills.
- Fluency in Spanish (bilingual, strongly preferred).
- Experience in a performance-based culture with metrics attainment (preferred).
- Experience in the developmental disability field (preferred).
- Proficiency in Microsoft Office & Google Suite (preferred).
- Knowledge of California developmental disability systems (preferred).
- Reliable high-speed internet and quiet workspace.
Responsibilities
- Execute person-centered planning using specialized facilitation frameworks.
- Translate personalized plans into concrete support needs and advocate for them.
- Manage project timelines for clients and partner organizations.
- Leverage internal resources to connect clients with service providers.
- Contribute client insights to company strategy and initiatives.
