Requirements
- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in Illinois
- Credentialed Medicaid provider for the state of Illinois
- Prior experience performing wellness visits
- Professionalism, integrity, accountability, and compassion
- Ability to work autonomously and in a team environment
- Technologically savvy with charting tools and HIPAA secure messaging
- Strong written and verbal communication skills
- Pass national background check and valid clinical license search
Responsibilities
- Review daily caseload and prepare necessary technology.
- Navigate daily schedule and partner with in-home clinical team members.
- Conduct virtual adult and pediatric wellness visits and health assessments.
- Perform health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education on health and well-being.
- Collaborate with in-home team for vital signs evaluation and blood draws.
- Coordinate care, including medication management and specialist referrals.
- Maintain high-quality documentation of patient findings.
