Requirements
- Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
- Minimum of 3 years of recent clinical experience.
- Substantial experience in hospice, palliative care, and/or acute inpatient care.
- National certification, licensure, and prescriptive authority for advanced nursing practice.
- Must obtain licensure and prescriptive authority in all required jurisdictions.
- Must hold and maintain Basic Life Support (BLS) certificate.
- Must hold a compact RN license in their home state by date of hire.
- Proficiency with Microsoft Office programs (Word, Excel, PowerPoint).
- Ability to learn and efficiently use patient health information systems.
- Must maintain a HIPAA compliant, temperature controlled, noise-free home office.
Responsibilities
- Provides fully remote virtual serious illness care management to patients and families.
- Consults with medical staff and IDT members to ensure optimal patient care.
- Performs virtual assessment of patient, family, and caregiver needs.
- Identifies, manages, orders, and prescribes medication and equipment.
- Adjusts and supports the plan of care in accordance with policies and nurse practice act.
- Provides education to increase caregiver confidence.
- Supports staff by answering clinical questions and entering orders/prescriptions.
- Documents interactions in the electronic medical record (EMR).
- Participates in QAPI activities and medical record reviews.
