Requirements
- Current RN and APRN or Physician Assistant license in AL, AZ, GA, IL, OH, or TX.
- Bilingual fluency in Spanish and English.
- 5+ years of experience in cardiology clinical practice.
- 2+ years of experience in an outpatient cardiology setting.
- Proficiency with telehealth video platforms and virtual care workflows.
- Highly comfortable with EMRs, messaging platforms, and online portals.
- Strong understanding of managed care, HEDIS measures, and clinical coding.
- Ability to work 8-5 CST or later time zones (MT, PST).
- Secure workspace with reliable internet connection.
Responsibilities
- Conduct virtual clinical assessments for general cardiology and heart failure.
- Develop and execute individualized treatment plans including medication management.
- Initiate and titrate guideline-directed medical therapies (GDMT).
- Monitor and manage acute volume overload and weight fluctuations.
- Order in-home diagnostics like echocardiograms, EKGs, and Holters.
- Collaborate with interdisciplinary teams including PCPs, cardiologists, and pharmacy.
- Serve as clinical advisor and escalation point for specialty clinic staff.
- Maintain accurate patient records and ensure regulatory compliance.
