Accountable for patient outcomes and KPIs for CKD and ESKD patients. Obtain patient history, perform physical exams, order and interpret diagnostic tests. Formulate plans for individual patient needs. Present patient cases and provide clinical support in meetings. Ensure care plans incorporate best practices for chronic kidney care. Participate in shared decision-making and end-of-life planning. Serve patients in multiple care settings including home, clinics, and via telehealth. Liaise with dialysis facilities, PCPs, and nephrologists. Maintain board certification and continuing education requirements. Adhere to documentation policies. Deliver primary and urgent care visits to adult and geriatric patients.
