Requirements:

• A minimum of two (2) years of related experience, preferably in an outpatient clinic setting.
• CMA certification preferred.
• Current Basic Life Support (BLS) certification.
• Effective communication skills.
• Flexibility, self-motivation, and excellent interpersonal, communication and organizational skills.

Responsibilities:

• Collaborate with primary care teams to manage chronic disease patients using the Phamily platform.
• Provide courteous interactions and accurately direct patients to appropriate clinical staff.
• Organize patient data, identify unmet needs, and enhance communication between patients and their care teams.
• Engage in quality improvement and support care redesign efforts.
• Manage a caseload of 500 patients with 300 billable by the end of the month.
