Requirements:
  • Bachelor’s Degree in Human Services (preferably social work, psychology or counseling). Master's degree and license preferred.
  • Minimum of 2 years of experience with the population.
  • Knowledge of program policies and procedures.
  • Preferred experience in Medicaid/Public Sector services.
  • Proficient understanding of person-centered planning principles.
  • Familiarity with computer applications and multi-screen navigation.
  • Demonstrated ability to uphold confidentiality standards and ensure HIPAA compliance.
  • Strong organizational skills and a sense of urgency to meet deadlines.
Responsibilities:
  • Schedule, conduct and document member assessments within prescribed timelines.
  • Enter assessment data into the web system.
  • Participate in inter-rater activities for consistency in assessment instruments.
  • Engage in monthly Waiver staff meetings and necessary training.
  • Support department procedures and quality assurance initiatives.
  • Conduct educational sessions for families and agencies regarding assessments.
  • Complete satisfaction surveys to achieve positive results.
