Requirements:

• Registered Nurse, BSN prepared, current unrestricted multi-state (compact) license.
• Fluent in Spanish required.
• 4+ years experience in a hospital and/or healthcare practice, serving maternal-child health populations (minimum 3 years in labor and delivery).
• Flexible and agile thinker who embraces change.
• Internet-connected, able to work remotely via video, phone, and text.
• Willing to occasionally travel.
• Willing to work weekends and holidays.
• Understanding of birth inequity and structural racism's role in maternal morbidity and mortality.

Responsibilities:

• Ongoing clinical and psychosocial assessment of new patients, providing reassurance and building rapport.
• Establishing individualized care plans to meet specific health needs based on evidence-based standards.
• Developing wellness goals by determining motivating factors.
• Identifying and addressing barriers to care.
• Reducing care gaps by frequent patient engagement.
• Assessing the need for remote health monitoring equipment.
• Ongoing surveillance and timely response to abnormal diagnostic results.
• Assessment of urgent concerns and triaging patients appropriately.
• Timely documentation of care interactions.
• Supporting program and product development by providing user feedback.
