Requirements:

• 3+ years' experience as a people manager including coaching and development.
• 3+ years' experience in delivering care to patients with CKD, ESKD, or other chronic diseases.
• Must hold an active, unrestricted clinical license as RN, SW, RD, PharmD, PA, or NP.
• Bilingual with Spanish and English proficiency.
• Must reside locally in Puerto Rico.
• Bachelor's or Master's degree in Nursing, Social Work, Nutrition, PA Studies, or Doctor of Pharmacy.
• Excellent leadership skills for cross-functional team collaboration.
• Strong ability to use data for performance improvement and problem-solving.

Responsibilities:

• Provide leadership for interdisciplinary team engagement and performance improvement.
• Design, operationalize, and monitor clinical workflows.
• Partner with clinical directors for value-based care delivery.
• Foster a culture of clinical excellence and quality.
• Identify and implement strategies to streamline operations.
• Lead continuous quality improvement projects with data analysis.
• Mentor clinical supervisors and staff to achieve professional development.
• Monitor performance and support supervisors in meeting targets.
• Collaborate with internal stakeholders to align on goals.
• Drive quality assurance measures for enhanced patient care outcomes.
• Maintain and oversee clinical operations policies and governance.
• Ensure compliance with healthcare regulations and mitigate risks.
• Recruit and train clinical supervisors and teams aligned with company values.
