Licensed Practical Nurse, Chronic Care Management

Requirements:

• Multi-state LPN Compact State Licensure is required.
• 5+ years of clinical experience in chronic care management required.
• Previous experience leading or building a CCM program.
• EMR experience, preferably in Athena and EPIC.
• Strong communication and collaboration skills.

Responsibilities:

• Support the Chronic Care Management Program to better manage patients’ conditions.
• Partner with Operations and Clinical Leadership team to integrate the program into the current RPM program.
• Provide care, education, and support for CCM patients.
