Requirements:

• 5+ years experience in treating chronic diseases (T2D, Hypertension, CHF).
• Active multi-state compact RN license and active NP license in either IN, IL, TX, or NM.
• ANCC or AANP certification required.
• Master’s Degree as a Nurse Practitioner with a willingness to expand state licensure.
• Experience in managing CHF with guideline-directed medical therapy.
• Stable home office setup with reliable wifi for remote work.

Responsibilities:

• Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs.
• Manage treatment plans and medications with emphasis on guideline-directed medical therapy.
• Respond to patient escalations identified via remote patient monitoring.
• Lead regular virtual check-ins with patients to review labs and optimize treatment adherence.
• Ensure exceptional healthcare interactions prioritizing patient well-being.
• Support internal growth efforts and shape team culture.
• Participate in quarterly compliance audits and meetings.
